A Notice to Provide Information or written request is required and should include details about client information such as name, PHN or DOB. Requested information should be listed separately- Health information disclosure under HIA and documents to be released under FOIP.
- A Notice to Provide Information or written request requires citing the appropriate section of the Health Professions Act and FOIP (if applies) and agreement in writing to not disclose the information to any other person except as authorized by or under the governing Act.
- A copy of the Directive letter to the investigator is required to validate authority of requester.
