Carewest programs and services cover a range of care including long-term care, rehabilitation and recovery services, and community programs. Our goal is to meet the needs of the community through quality care while effectively and efficiently responding to an ever-changing health care system.

How to access our services

AHS Transition Services Coordinators act as navigators to the community services and resources by linking callers to appropriate resources.

Anyone may make a referral by calling Transition Services – family, friend, health care professional or a client. Transition Services Coordinators will refer you to Home Care who will assess you for the appropriate community service. The client must be in agreement with the referral and consent to an in-home assessment. To reach Home Care, please call 403-943-1920.

Dementia Care Training – Supportive Pathways Program

Carewest’s Supportive Pathways program is shared province-wide in a Train-the-Trainer format after Alberta Health and Wellness selected it as the provincial model of dementia care training. It is offered to educators / managers and other professionals who work with clients / residents who have dementia within Carewest and throughout the province and country.
