Comprehensive Community Care (C3) Program is a unique program for frail seniors with chronic illnesses who live in designated areas of Calgary and require extra supports to live safely at home. If you need ongoing medical care, day support, and/or home services to remain independent at home, the C3 program could be for you.
Integrated care is provided by an interdisciplinary health care team, which includes a medical health clinic, day program, home support, transportation assistance, and access to a designated treatment bed. Our objective is to partner with you to manage your physical, medical and social well-being. We can assist you with medications, laboratory tests, specialist appointments and therapy.
Alberta Health Services Integrated Home Care will help you with managing your personal care (such as bathing, getting dressed, taking medications, etc.).
C3 will assist you with arranging transportation to and from Carewest Beddington C3 or Carewest Sarcee C3 two days a week where a morning snack and lunch is provided. Our team of health care professionals will support you to stay as healthy and independent as possible. You may be seen by any members of the care team (depending on your needs). To help manage your health and well-being, and deliver continuity of care in a timely manner, we provide our own physicians. This means that once you join the program, one of our family physicians will direct your care. Our pharmacy will provide you with medications.
Social opportunities are an important part of the C3 Program. The Day Centre provides an ideal environment to make friends and take part in meaningful activities.
Carewest Beddington and Sarcee C3 has access to one short-stay bed at Carewest Sarcee that is available for medical treatment. It is a potential option should you require short-term medical treatment if you are too ill to stay at home.
Costs
There is a minimum monthly fee for the C3 program. Subsidy may be available to help cover the costs based on eligibility.
Medication costs are determined by our Pharmacy and will be processed monthly.
How to Join
- Eligibility is determined by age, desire to remain in the home, among other factors determined by a Home Care Nurse.

Locations
Both the C3 Program locations are open Monday to Friday between 8 a.m. and 4 p.m. Most clients arrive around 9:00 a.m. and leave around 2:30 p.m.
